Case Manager (Bilingual in Russian/English speaking) - UJA Safety Net
Auto Import<strong>Position Summary</strong>: Assesses clients' needs and eligibility for in-home services, entitlements and benefits, and manages ongoing case work and care plan management.<br><br><strong>Principal Responsibilities<br><br></strong><strong>Knowledge, Skills And Requirements<br><br></strong><ul><li>Conduct intakes, provide accurate information and referrals.</li><li>Screen and assist clients with applications and re-certifications for entitlements and benefits</li><li>Assess clients in their home, hospital or other institutional setting as appropriate</li><li>Advocate on behalf of clients and arrange for provision of services; develop implement and follow up on care plans</li><li>Provide supportive counseling to clients as needed</li><li>Maintain ongoing communication with client, family members and other collaterals as appropriate</li><li>Maintain complete and accurate case notes utilizing the program’s data system; prepare required documentation in case file within organizational time frames</li><li>Maintain and report on statistics according to Selfhelp standards, utilizing the program’s data system and other forms of record keeping as needed.</li><li>Actively participate in supervision and staff meetings and share any emergent issues with supervisor in a timely manner</li><li>Facilitate and assist at social, recreational and other group activities and events</li><li>Any other duties assigned. </li><li>Adhere to organization’s standards by completing annual mandatory trainings in a timely manner on topics including – but not limited to – Sexual Harassment Prevention Training, IT Security Awareness, Compliance, OSHA, HIPAA and any other training the organization may deem necessary in regards to regulatory compliance or good business practices.</li><li>Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.</li><li>Adheres to the organization’s policy in regards to absenteeism and appearance and health and safety standards<br><br><br></li></ul><strong>Salary Range: $54,000 - $56,400 per year commensurate with experience.<br><br></strong><strong>Job Competencies & Minimum Qualifications<br><br></strong><strong>Knowledge, Skills And Requirements<br><br></strong><ul><li>Bachelor's Degree Required.</li><li>Russian/English Speaking required</li><li>One year related experience and/or training</li><li>Working knowledge of Microsoft Office Suite and other technology</li><li>Excellent communication and listening skills</li><li>Excellent customer service skills with a focus on treating clients with respect and dignity</li><li>Able to multi-task and work independently with a great attention to detail<br><br><br></li></ul><strong>Working Conditions/Physical Demand<br><br></strong><strong>Knowledge, Skills And Requirements<br><br></strong><ul><li>Business office environment with phone and computer use; travel via public transportation. This position may have the ability to work from home 2-3 times per week.<br></li></ul>
